> Fat burners carry real risks and are a higher-risk supplement category. In South Africa they are sold as unregistered Category D complementary medicines, not evaluated by SAHPRA, and some contain high caffeine doses. Banned or controlled stimulants have appeared in this category internationally. We could not verify the legal status of every ingredient, so check labels, avoid them if you have heart problems, and ask a pharmacist.

Key facts:
- South African fat burners are sold as unregistered Category D medicines that Dis-Chem pages state are not evaluated by SAHPRA.
- EFSA 2018 linked 800 mg or more of EGCG a day from supplements to raised liver enzymes.
- Duiven 2021 found undeclared doping substances in 38% of 66 Dutch web-shop fat-loss and energy products.
- Biogen Rage Thermo Extreme's label dose (2 capsules twice daily at 150 mg each) totals 600 mg caffeine a day by our calculation.

## Are fat burners legal in South Africa?

They are sold, but "sold" does not mean "evaluated". On the Dis-Chem Living Fit pages we read for USN PhedraCut, Biogen Rage Thermo Extreme and NPL Thermo Cuts, each product is described as having not been evaluated by SAHPRA. Under the current framework, as of August 2026 SAHPRA had registered no complementary medicines, so unregistered products can be on shelves with a disclaimer ([are supplements regulated in South Africa?](/answers/are-supplements-regulated-in-south-africa/)).

Ingredient-level legality is a separate matter, and here we must be careful:

- **Ephedrine and related substances.** A government notice under the Medicines and Related Substances Act is titled as a scheduling notice for ephedrine, pseudoephedrine and D-nor-pseudoephedrine. We could read only the notice title, not the schedule details, so we do not state which schedule applies; ask a pharmacist about any product that lists these ingredients.
- **DMAA and similar stimulants.** Dis-Chem pulled the pre-workout Jack3d from shelves in November 2015 (the article body was unreadable to us, so we claim nothing further). DMAA (1,3-dimethylamylamine) is named in Schedule 7, the most restrictive schedule, in the Acts Online text of the Medicines Act schedules (substituted by a notice dated 1 August 2025), so it cannot lawfully be sold as a supplement ingredient. We found no 2026 SAHPRA recall notice for DMAA or DMHA products. The AIS lists DMAA, DMBA, synephrine and ephedrine in Group D.

## What are the main safety risks?

| Risk | What happens | Evidence |
|---|---|---|
| Stimulant overload | Palpitations, raised blood pressure, anxiety, insomnia | Established; dose-related |
| Liver injury | High-dose green tea extract; EFSA found 800 mg a day or more of EGCG linked to raised liver enzymes | EFSA 2018; UK Committee on Toxicity 2024 |
| Undeclared drugs | 38% of 66 Dutch fat-loss, muscle and energy products contained undeclared doping substances | Duiven 2021 |
| Drug interactions | Antidepressants, stimulant medicines, thyroid medicines | Established clinical caution |

Caffeine labelling in South Africa is inconsistent. Biogen Rage Thermo Extreme states 150 mg per capsule with a dose of 2 capsules twice a day (600 mg a day by our calculation). USN Slim Packs list 170 mg per sachet on the USN shop but 150 mg on a Dis-Chem page. Slimz One-A-Day has 90 mg per capsule. Pages also say that over 200 mg of caffeine a day is not recommended in pregnancy. Add up all sources, including coffee and pre-workout.

## Who should not use them?

Heart disease, high blood pressure, anxiety disorders, thyroid disease, pregnancy and breastfeeding, anyone under 18, and anyone taking antidepressants (such as MAOIs or SSRIs) or stimulant medication. Stop and seek help for chest pain, fainting, severe headache, yellow skin or eyes, or dark urine.

## What about drug-tested athletes?

SAIDS singles out stimulant "fat burners", pre-workouts and some botanicals as higher risk, and applies strict liability: you are responsible for whatever is in your sample ([can supplements cause a failed drug test?](/answers/can-supplements-cause-failed-drug-test-south-africa-rugby/)). Third-party batch testing lowers but does not remove the risk. The retailer pages for some products state Informed Sport or Informed Choice, but we did not check any against the certification registers.

## Is there a safer way?

Yes: a modest energy deficit, enough protein and resistance training do more than any capsule. If you want caffeine, coffee is easier to dose. See [do fat burners work?](/answers/do-fat-burners-work-south-africa/) and [what supplements are banned in South Africa](/answers/what-supplements-are-banned-in-south-africa/). If you decide to try one anyway, speak to a pharmacist first, begin with half a dose and stop at the first sign of palpitations.
