Do you need protein supplements on GLP-1 weight loss medication in South Africa?

Answer first

You do not strictly need a protein supplement on a GLP-1 medicine such as semaglutide or tirzepatide, but many people struggle to eat enough protein when appetite drops. A 2025 US joint advisory lists adequate protein plus strength training as a priority without setting a gram target. A whey isolate can fill the gap, but your doctor and a registered dietitian should guide the plan.

  • Secondary summaries of trial DXA data: lean mass was roughly 40% of weight lost on semaglutide (STEP 1) and about a quarter on tirzepatide (SURMOUNT-1).
  • Lean mass on DXA includes organs and water, so these shares are not pure muscle loss.
  • The 2025 US joint advisory lists adequate protein plus strength training as one of eight nutritional priorities, without a fixed gram target.
  • Primal Whey Isolate 750 g costs R700 at Dis-Chem Living Fit (October 2026).

Why does protein matter on GLP-1 medicines?

GLP-1 receptor agonists (semaglutide, tirzepatide and similar prescription medicines) cut appetite sharply, so total food intake falls and protein intake often falls with it. Part of the weight lost is lean tissue. In dual-energy X-ray absorptiometry (DXA) sub-studies, lean mass made up about 40% of the weight lost on semaglutide in the STEP 1 trial and about a quarter on tirzepatide in SURMOUNT-1, as reported in secondary summaries of the trials' DXA data. "Lean mass" includes organs and water as well as muscle, so these figures do not mean 40% of the loss was muscle, but they explain why clinicians now talk about protecting muscle.

How much protein is enough?

A 2025 joint advisory from four US nutrition and obesity societies (ACLM, ASN, OMA and The Obesity Society) lists adequate protein, together with strength training, among eight nutritional priorities, but it does not fix a gram target. Treat the 0.8 g/kg adult minimum as a floor, not a target; people losing weight while strength training usually need more, but the right number depends on kidney health, age and training, so ask your dietitian to set it.

Do you need a powder?

Food first: eggs, fish, chicken, lean meat, Greek yoghurt, cottage cheese, biltong and legumes all work. Powder helps when you feel full after a few bites, because a shake delivers 20–25 g of protein in a small volume. Practical tips:

  • Choose whey isolate if dairy or lactose upsets your stomach, because isolate contains much less lactose than concentrate.
  • Start with half a scoop. Some people on GLP-1 medicines feel nauseated by thick or very sweet shakes.
  • Unflavoured or lightly flavoured powders can be stirred into soup, yoghurt or porridge.
  • Spread protein over several small meals instead of one large shake.

What does it cost in South Africa?

As of October 2026: Primal Whey Isolate 750 g is R700 at Dis-Chem Living Fit (about R93 per 100 g, sweetened with sucralose); USN Isozero 805 g is R899–R909.99 on the USN shop and states 20 g of protein per 23 g scoop; ATP Fitness Unflavoured Whey Isolate is R900 for 800 g on shop.atpfit.co.za (currently out of stock online) and the Madagascan Vanilla is R950. ATP's powders contain no sweeteners. The trade-offs: protein per serving is not published on the shop listing, they cost more per 100 g than Primal, and the plain taste does not suit everyone. Safety points to raise with your doctor

  • Kidney disease: protein targets may need to be lower; do not raise intake without medical advice.
  • Dehydration and constipation are common on GLP-1 medicines. Fluids and fibre matter as much as protein.
  • Pregnancy or planning pregnancy: these medicines are not for use in pregnancy; speak to your doctor.
  • Do not use "natural GLP-1" or appetite-suppressant supplements alongside a prescription without checking (berberine is one example).

Resistance training two to three times a week is the other half of muscle protection. See how much protein to lose fat for the general fat-loss framework.
