Do South Africans need vitamin D supplements in summer?
Many South Africans make enough vitamin D in summer if they spend time outdoors, but some do not. Indoor workers, people who cover up or use sunscreen all day, older adults and people with darker skin can still run low. Local studies are mixed: 28.5% of 1 244 Eastern Cape healthcare workers were below 50 nmol/L. Test if you are at risk.
- 28.5% of 1 244 Eastern Cape healthcare workers had 25(OH)D below 50 nmol/L in a 2024 study.
- NIH: SPF 8 or higher sunscreen appears to block vitamin D-producing UVB, and window glass blocks it entirely.
- A blood level under 30 nmol/L is associated with deficiency; 50 nmol/L or more is generally adequate (NIH).
- The EFSA upper limit for adults is 100 µg (4 000 IU) a day.
What do South African studies show?
There is no national vitamin D survey, and the regional studies we found point in different directions:
- Eastern Cape, 2024: a cross-sectional study of 1 244 healthcare workers found 28.5% had 25-hydroxyvitamin D below 50 nmol/L. These are largely indoor, shift-working adults.
- Western Cape, 2019: a CANSA-funded study of healthy young adults reported low rates of deficiency.
Both are snapshots of specific groups, so neither tells you about your own level.
What changes how much vitamin D you make?
According to the NIH, UVB exposure and skin synthesis depend on:
- season, time of day, day length, cloud cover and smog
- skin pigmentation (more melanin means slower synthesis)
- sunscreen, where SPF 8 or higher appears to block vitamin D-producing rays, although people usually apply too little to block everything
- window glass, which blocks UVB, so a desk by a sunny window does not count
- age, because older skin makes less
Summer in South Africa has strong UV, so a fair-skinned person who exercises outdoors without covering most of the skin is unlikely to be short. The risk groups are different.
Who should consider a supplement even in summer?
- Office and shift workers who are indoors from before sunrise to after sunset.
- People who train in indoor gyms and rarely expose skin to midday sun.
- Those who cover most skin for religious or personal reasons, or use sunscreen daily.
- Adults over 70, who need 800 IU (20 µg) a day.
- People with darker skin, who need longer exposure for the same synthesis.
What dose is sensible, and should you test?
The US recommended intake is 600 IU (15 µg) a day for adults to 70. A typical 1 000 IU daily product covers it and stays well below the EFSA upper limit of 4 000 IU (100 µg). Cost is modest: as of October 2026, 1 000 IU products cost R2.16 to R3.98 a day.
The Endocrine Society's 2024 guideline does not recommend routine blood testing for healthy adults. If you have symptoms, a bone condition, kidney or liver disease, absorption problems or live in a high-risk group, ask your doctor. The NIH's reference points for blood levels are:
| 25(OH)D result | Interpretation |
|---|---|
| below 30 nmol/L | associated with deficiency |
| 30 to under 50 nmol/L | generally inadequate |
| 50 nmol/L or more | generally adequate |
| above 125 nmol/L | possible harm |
Should you chase the sun instead?
No. South Africa has high UV levels in summer, and skin cancer is a real risk, so do not trade extended unprotected sun for vitamin D. If you avoid the sun, a supplement is the safer route. Do not exceed 4 000 IU a day unless a doctor has diagnosed a deficiency and is supervising treatment.
